Horizon Medical provides comprehensive medical care within assisted living facilities and nursing homes, allowing residents to receive treatment without leaving the comfort of their apartments. Our service involves regular visits from healthcare providers at scheduled times. Residents can simply register with the facility’s designated representative to receive medical “house calls” during our visits.

Insurance Coverage
We accept all insurance payers operating in Mississippi, including TriCare.
Prescription Management
We manage prescription refills for chronic conditions for our
primary care patients.
Immunizations
We administer annual vaccines, including flu, COVID-19, shingles, pneumonia,
and TB testing.
Health Assessments
We perform annual Health and Physical exams (H&Ps) for residents, with
a proactive care model emphasizing regular visits every 60-90 days.
Communication
We maintain communication with a family member or healthcare advocate
designated by each resident.
Medicare Annual Wellness Visits
We administer annual vaccines, including flu, COVID-19, shingles, pneumonia,
and TB testing.
Laboratory Services
We conduct laboratory tests on-site, eliminating the need for residents to
leave the facility.
Primary Care
Our team provides primary care services directly within the facility, managing
both chronic and acute conditions without requiring residents to leave the premises.
Specialist Referrals
Residents are encouraged to continue seeing their specialists. We are
happy to provide referrals to additional specialists as needed.
Regular Visits
Providers will be scheduled to visit the facility at least once per week, with the
frequency potentially increasing to twice per week based on the facility’s needs and resident numbers.
Telehealth Services
For issues arising on days when we are not on-site, we offer telehealth
consultations.
